The preadmission process at Extended Hands is an important step towards
appropriate placement. A conscientious review of your loved one's
needs will include:
- touring the facility grounds;
- providing a copy of the resident's rights;
- a physician evaluation including a current TB skin test;
- completion of a preadmission appraisal;
- clarification and provision of any advance directives by the health
provider or responsible party;
- discussion and initiation of an initial service plan;
- completion of an activity and lifestyle assessment;
- coordination of a move-in date and plan to minimize transfer
trauma.
At Extended Hands, we encourage the entire family to be a part of the
admission process so everyone feels comfortable with the placement.
The resident is encouraged to visit prior to placement.
